Q: Is the output connection size 1 inch?

Inlet and outlet are both male POL (left hand thread)

Q: How can this work if both sides screw into the output port? Should one fitting connect to the input port of giving tank so it will draw from bottom?

One end screws into output of large tank; (500 gal in my case); and other end screws into small tank. I use the end with handle shutoff at the small 5 gal/20lb bottle so when I shut it off the hose is still full for a 2nd and 3rd tank. You need an adapter with shutoff at the 500 gal tank so you can screw this hose into it. Do not use your hookup for house to fill with. There is another outlet to screw the adapter into. Good luck.
