Q: Is this keypad only the rubber part or Does it include the circuit pad underneath the push buttons

I don't know. I purchased it and the maintenance staff at my father's care facility installed this item. It is my impression that it includes a circuit board but I'm not 100%.

Q: My push pad ( on off ) does not work . i push it and it does not lite up or make a noise . what do i need to do now

The cheapest route is to replace the keypad. If it works it save you a whole lot of money. If not your joystick burnt out and the joystick need to be replaced. 1-888-317-9789

Maybe. Probably not. This is simply a push pad that sticks on the arm of the Jazzy. You peel the old one off & clean the contacts underneath with alcohol & qtip. You can stick the pad back on. If it tears you will need this one to stick back on. It has self adhesive on the back. My wife's Jazzy is over 10 years old. This pad fit perfectly and worked after cleaning the black stuff off the contacts. The joy stick is separate from this. Sounds like you may have an electrical problem.
